This interdisciplinary team consists of a care manager, caregiver, and supervisory nurse, as well as any other provider who is involved with patient care. This includes their family members, who are granted access to the Patient Portal from anywhere.

Supervisory Nurse
Experience peace of mind knowing your loved one will have a dedicated supervisory nurse overseeing his/her care every step of the way. With up to six personalized visits annually, the nurse will ensure the care plan is being followed and communicate updates with the interdisciplinary team (IDT).
Dedicated Care Manager
The dedicated care manager will oversee and coordinate your loved one’s comprehensive care plan, ensuring they receive all necessary medical services and support. You can contact your care manager 24/7 regarding any patient needs or concerns.
Hospitalization Support
If your loved one is admitted to the hospital, a supervisory nurse will visit your loved one in the hospital within 24 hours and provide a report to all parties about his/her medical status through the portal. (A separate sign up is required for this service.)

Join Appointments via Microsoft Teams
You can schedule meetings or calls with the supervisory nurse or even join appointments virtually to understand how your loved one is doing. These calls are recorded and shared on the portal if you’re not able to join in real time.
